Academic Integrity Violation Notification Form (A&B) - given to student when alleged violation is discovered. - DOC | PDF

Academic Integrity Resolution Form (C) - completed by instructor, student, & Academic Integrity Facilitator after their meeting. - DOC |PDF

Forms A, B, & C must be completed and returned to the Academic Integrity Assistant at the conclusion of the resolution meeting in order for the charge to be official. (Also see "instructions for notifying the student" link above.)

Academic Integrity Appeals Forms - completed and turned in to 101 Whitehurst within five days of the resolution meeting.
